Traditionally, treatment of diabetes includes 3 main approaches: Diet & Lifestyle modifications, Oral medications (Sulfonylureas, Biguanides, Glitazones etc.) and Insulin.

Though these are quite effective, they are nowhere near mimicking the physiological control of blood sugar in a healthy person. (I will write about why current medications cannot match the physiological control in another topic.)

A new drug class to the list:

The USFDA (United States Food and Drug Administration) has recently approved a new medication for treatment of type 2 diabetes - called EXENATIDE. The brand name is BYETTA and is a product of Amylin-Lilly.

Exenatide is a medication given as injection. It belongs to a new group of anti-diabetic drugs called incretin mimetics. This drug derived from a poisonous Gila monster, a poisonous lizard (see picture under this post, click on the image to enlarge it) that lives in the Southwest United States and Mexico.

Exenatide injection is used in combination with metformin and/or a sulfonylurea medication to treat type 2 diabetes.

What are the mechanisms of action for Exenatide?

  • Stimulates insulin secretion from the beta cells of pancreas when blood glucose is high
  • Restores 1st phase insulin response (first 10 minutes after food ingested) that is specifically lost early in type 2 diabetes
  • Lowers production of Glucagon after meals
  • Slows gastric emptying which limits rise in blood glucose following a meal
  • Reduces caloric intake causing a weight loss

This arms the desperate diabetician with another weapon to use against diabetes, but it is still NOT the perfect answer.

How is this new drug in comparison with insulin?

Is it more effective? Have we found out a new alternative to insulin?

I do not think it is available for therapeutic use at present. However, some centers may be using it for research purposes.

In comparison to Insulin, advantages are that Exenatide cause significantly less hypoglycemia and and results in weight loss. On the other hand, it does cost more, but the patients saves on the incidences of hypoglycemia, which could be expensive at times.

However, it is far from being an alternative to insulin. It is approved for use in type 2 diabetes alone and insulin is still the drug of choice for type 1 diabetes.

sir, how about the side effects of the exenatide? in the higher dose or if the patient is not taking their meals properly, will it cause hypogycemic coma?

As mentioned in the first post, one of the mechanisms is "Stimulates insulin secretion from the beta cells of pancreas when blood glucose is high" unlike sulfonylureas that stimulate insulin release irrespective of blood glucose levels. Hence, hypoglycemia is not common with Exenatide alone.

However, Exenatide is always used in type 2 DM in conjunction with either sulfonylureas or metformin. In combination with these drugs, hypoglycemia may be observed often. Check out this PubMed Search for Exenatide & hypoglycemia.

Relative incidence of hypoglycemia with Exenatide:
Exenatide with Sulfonylureas:
1. Placebo - 3%
2. 5mcg/dose Exenatide - 14%
3. 10mcg/dose Exenatide - 36%

Exenatide with Metformin & Sulfonylurea

1. Placebo - 13%
2. 5mcg/dose Exenatide - 19%
3. 10mcg/dose Exenatide - 28%

More important than hypoglycemia are gastrointestinal adverse effects. Though Exenatide is given subcutaneously, it causes significant GI adverse effects - nausea and vomiting. Most patients who discontinue this medication do so due to the GI adverse effects.

Why is Exenatide preferred over insulin?
1. Milder and less frequent hypoglycemia
2. Weight loss (as opposed to weight gain with insulin).
